Sackhead Girl is a character in BLACKSOULS 2 you can find in one of the rooms in the Crimean Nursing Graveyard, only in DLC3. She’s an NPC locked to a chair you can assault if you want to, it’s totally optional.

The Sackhead Girl appears only in the DLC3 part of the second game.
She’s locked on the ground floor in one of the rooms to the left, and there she’s pretty much entirely helpless.
Apparently, Florence takes care of her every day while she’s locked there. The Sackhead Girl will also mention how Florence ran the medical institutions by herself while she was still located in Wonderland. She talks a lot about Florence and how much she’s dedicated to taking care of her patients.
If you want to talk more to the Sackhead Girl, she will ask you to bring her some viscera in exchange for more stories. You can get the viscera from Dr. Jekyll on the second floor.
In exchange for viscera, she will give you the Wittled Ring.
If you talk to her more, she will just be asking for more viscera.
During a forced NSFW scene with her, you can put a picture of Alice on her head, all to satisfy your “Alicephilia”.
